Though I am not a doctor I can answer this question. I worked in the top Pharma company for more than two and a half decades and I have seen the doctors day in and day out in various positions.
I have seen housing Surgeons and PGs becoming the top Physicians, Cardios, Gastros, and Endos (I was in the cardiac and Diabetic division).
