By in-service, I think you mean the doctors who are working at hospitals after their MBBS.
I have spoken to some doctors who had taken up jobs after their MBBS. The general sense I get is that the jobs are demanding and do not allow enough time for preparation. So they generally quit their jobs a year in advance in order to prepare.
